Jake's Blues receives many requests throughout the year to participate in charitable functions. While Jake's Blues tries to support events that help raise money for worthy causes, we now require the submission of a standard form with information about the event and responsible parties for our tax and general record keeping purposes.

For charitable gig requests, please download a request form HERE and either scan and email back to us, or US Postal Mail it back to Jake. The address is listed on the form. Sorry, we do not accept FAX at this time.
